Technology/Acceptable Use/3rd Party Apps Forms

All DCS students must complete the Acceptable Use Policy and 3rd Party Agreement Form. Students issued a DCS Chromebook should complete both forms in the linked files below:

The following form may be used for students who are not issued a personal device, but must still agree to DCS Acceptable Use Policy. In addition to this form, they will still need to complete the 3rd Party Agreement form from the documents linked above:
